IMPORTANT: 2025 Health Coverage Tax Documents (Form 1095-C)

In accordance with the federal Paperwork Burden Reduction Act, The Catholic Diocese of Saginaw and the Diocese of Saginaw Catholic Cemeteries is not automatically mailing paper copies of the 2025 Form 1095-C to all employees. These forms, which contain information about your health insurance coverage, are available upon request.

How to Request Your Form

The Form 1095-C can be viewed and printed from your Paycor login.
You can also receive a copy of your 2025 Form 1095-C by contacting Melissa Seeger in the Finance Office or by:

Requests will be fulfilled within 30 days. This notice will remain posted through October 15, 2026.

Reimbursement Forms

The Diocesan mileage reimbursement rate has changed effective September 2025 to $.70 per reimbursable mile.  All Diocesan staff are asked to use a Diocesan vehicle when traveling on Diocesan business if possible and practical.
